The North Carolina Dental Society Foundation works to promote the oral health of children and adults across our state through education, prevention, and free dental services. Supporting the oral health of our neighbors in need is one way we are  seeking to close the healthcare gap in North Carolina.

We firmly believe that better oral healthcare leads to better overall health.

This year, we are holding a raffle to support the work of the NCDS Foundation.

Tickets are $100 each, and only 3,500 will be sold.

Terms and Conditions

  1. Proceeds

This raffle is a fundraising event, and all net proceeds benefit the North Carolina Dental Society Foundation (“Foundation”). The Foundation is a 501(c)(3) nonprofit organization.


  1. Tickets.

The cost to purchase a single entry (“ticket”) for the raffle is $100 and is not a tax-deductible contribution. One ticket equals one entry.


  1. Drawing

The drawing will take place on Friday, May 21, 2021. All purchases must be received by 5:00 PM EDT Friday, May 21, 2021. The random drawing will be conducted by an independent accounting firm, Hollingsworth Avent Averre & Purvis, PA. Raffle ticket purchases may be made by completing the online entry form at Entrants need not be present to win. Odds of winning depend upon the number of eligible entries received. The winner will be notified by email on Friday, May 21, 2021.


  1. Prize

The first winning ticket will be awarded the value of $75,000.00 that must be applied to the purchase of a new, luxury electric vehicle from a dealer the Foundation approves (“Vehicle Prize”).  The Foundation will withhold the amount of the required withholding taxes described in more detail in section 5(h) before the remainder is transferred to the dealer. The first winner is solely responsible for paying the approved dealer any and all amounts owed for the vehicle that are above the amount transferred to the dealer. The Vehicle Prize is not transferrable or redeemable for cash. However, the first winner may choose an alternative prize of $50,000.00 in cash, from which the Foundation will withhold the taxes described in more detail in section 5(h) (“Alternative Prize”).


If more than 3,000 tickets are sold by January 8, 2021, a second winning ticket will also be drawn. The prize for the second winning ticket is a cash prize of $5,000.00 (“Second Prize”).


The winner(s) will have ten calendar days to accept the prizes by notifying the Foundation.


If the first winner elects the Vehicle Prize, the Foundation will pay the approved dealer the amount of $75,000.00 less the amount of the required withholding taxes upon the following conditions:

(i)         the winner provides the Foundation with a complete and accurate form W-9;

(ii)        the winner selects a vehicle and confirms the order with the approved dealer within 30 days of the drawing; and

(iii)       the approved dealer notifies the Foundation of the order within 30 days of the drawing.


The winner is responsible for paying all taxes including, without limitation, any state highway use tax and all property taxes, sales, excise, or other taxes with respect to the Vehicle Prize and/or vehicle applicable to the buyer, as well as all registration, title, tag, and other fees. The winner is responsible for the costs of insuring the vehicle. The winner is responsible for paying all taxes applicable to the Alternate Prize and/or Second Prize, as applicable.


In the event the winner (s): (1) declines the prize; (2) is under 18; (3) lives in a state where the raffle is prohibited; (4) does not comply with the requirement to provide a W-9;  and/or (5) is ineligible to win for any other reason; the initial drawing will be deemed null and void, and the Foundation will draw another winning entry from among all the remaining eligible entries. Only two (2) alternate drawings will be held, after which the raffle will be cancelled, including cancelling of the award of the Second Prize, and the tickets will be refunded to the purchasers. The deadlines for accepting and disbursing the prizes will apply to the winner of the alternate drawing.


The winner(s) should consult with a tax professional about the obligations of winning the raffle.


  1. Rules


a) By entering this raffle, entrants accept and agree to be bound by all the rules, limitations, and restrictions set forth here, and that their names and/or likenesses may be disclosed to and used by the news media and may otherwise be used by the Foundation for publicity purposes without further compensation or right of approval, unless prohibited by law. All federal and state laws apply.


b) In these rules, the following terms are defined: An “entrant” is an individual whose name is entered on a ticket as the person eligible to win the prize. An entrant must be one natural person. A “purchaser” is the individual who buys a ticket naming him or herself, or another individual as the entrant. The “winner” is the entrant whose ticket is selected and who fulfills the requirements to receive the prize. There is no limitation on the number of tickets a purchaser can purchase.


c) Except as outlined below, all raffle ticket sales are final. The Foundation assumes no responsibility for lost, stolen, late, misdirected or non-delivered raffle tickets, mail, email, phone, fax, internet or other transactions or other messages, or any other failure to receive orders or deliver receipts prior to the drawing deadline. The Foundation reserves the right to cancel the raffle if a minimum of 3,000 tickets is not sold by Monday, March 15, 2021. In that event, all purchasers will receive a full refund of the price of their ticket(s), and the raffle will not occur.


d) This raffle is being conducted in North Carolina in accordance with North Carolina law without giving effect to any of its conflict of laws provisions thereof. By participating in this raffle, purchasers, entrants, and the winner further agree that any legal suit, action, or proceeding arising out of or relating to this raffle and these rules shall be brought exclusively in the applicable federal or state courts located in Raleigh, North Carolina. The raffle is void elsewhere where prohibited by law.


e) Each purchaser and entrant must be 18 years old or older at the time his or her ticket is purchased and must have a valid driver’s license. Employees of the Foundation and their immediate family members are ineligible to participate.


f) The Foundation does not make or provide any representation, guarantee, or warranty, expressed or implied, in connection with the prizes, and accepts no liability or responsibility regarding the construction or condition of the prizes.


g) By entering this raffle, entrants, purchasers, and the winner waive all right to, and hold the Foundation, North Carolina Dental Society, and North Carolina Services for Dentistry, Inc., Cvent, Inc., and Hollingsworth Avent Averre & Purvis PA, each of their respective affiliates, subsidiaries, directors, officers, employees, contractors, sponsors, advisors, agents, and representatives (together, “Released Parties”) harmless from, any claim, liability, loss, damage (including punitive, incidental, and consequential damages), or expense (including attorneys’ fees) arising out of or in connection with participation in this raffle or the acceptance, use, or misuse of any prize. The winner agrees to indemnify and hold harmless the Released Parties from any and all losses, damages, costs, expenses, rights, claims and actions of any kind arising in connection with or as a result of the winner’s acceptance or use of the prize, including (without limitation) failure to report, file and pay taxes due as a result of receipt of the prize. Released Parties will not be responsible for: late, incomplete, or incorrect entries; an entrant’s or purchasers’ failure to receive prize notices due to entrant’s or purchasers’ spam, junk e-mail, or other security settings or for entrants’ or purchasers’ provision of incorrect or otherwise non-functioning contact information; technical, hardware, or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled, or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or used in this raffle; by any human error which may occur in the processing of the entries in this raffle; or any typographical, technological, or other error in the publishing of the offer, administration of the raffle, or announcement of the prize. If, in the Foundation’s opinion, there is such an error, or there is any suspected evidence of tampering with any portion of the raffle, or if technical difficulties (including viruses and bugs) compromise the integrity of the raffle, the Foundation reserves the right to  cancel or modify this raffle in a manner deemed appropriate by the Foundation.


h) The Foundation is required by federal law to calculate the required federal income tax withholding due on the value of the prize, less the value of the wager. The Foundation is required by federal law to report the full value of the prize(s) as gaming income to federal and state authorities and to withhold, or collect, and deposit federal income taxes equal to 24% of that prize amount less the amount of the wager (i.e., the cost of the winning ticket). The Foundation shall deposit that amount and the winner will receive credit for the federal taxes remitted. The Foundation will provide the winner with a form W-2G.


Before disbursement of the prize, the winner must provide to the Foundation a completed and accurate form W-9. Upon disbursement of the prize(s), the Foundation will withhold the required amount from each prize awarded. For example, if the first winner selects the Vehicle Prize, the Foundation will withhold $17,976 for taxes, ((75,000 – 100) x .24), and the remainder shall be transferred to the approved dealer.


Each winner agrees to complete any other forms that the Foundation requires in order to comply with its legal and tax obligations.

The North Carolina Dental Society Foundation is committed to serving children and adults from underserved households through education, prevention and free-clinic settings. Through financial contributions and volunteerism, we work to improve oral health for North Carolinians.


We help North Carolinians in need – including children, veterans and adults. Some have never been to a dentist. From prevention and education to treatment, the Foundation focuses on individuals in dire need. More than 35% of children enter kindergarten with some tooth decay, and 21% of seniors statewide have lost all their teeth. We put smiles back on the faces of our neighbors.


Throughout the state, gifts to the Foundation from individuals, corporations, agencies and communities are transforming lives. People once reluctant to smile or talk because they were embarrassed about their teeth are now smiling again. Kids who couldn’t focus in school due to pain are healthier and learning.
